Just Google the state lottery office (of the state that your winning ticket is from) and there's ususlly a print-out form that you need to fill out when you send in your winning tickets.

Generally, even for a $1. or $2. payout, you need to send a copy of your drivers license, state ID or passport.

Thankfully many states just have a space on the back to fill out all your information, including drivers license and/or social security number.

But you're right, you may need to go to the states page. Thank you.

May I suggest not sending ones that you can win another ticket? I know a few in NY that you win a free ticket- I just feel as though the lottery office you send it to wouldn't bother sending you your free ticket.
